FREE Adult Family Home Placement Services:
This complimentary service for your family will include an interview with your family and your loved one, recommendations of appropriate adult family homes, accompanying you to adult family home interviews, advocating for your loved one's special needs during this transition and post placement case management for up to one month from the admission date to assure a successful placement for your loved one.
Washington State Long Term Care Assessments:
These assessments are required for all individuals moving into an adult family home. This assessment is used by the adult family home provider to assist them in developing a care plan for the new resident. Fee for this service is normally paid for by the family and/or the new resident. Cost: $200.00
Adult Family Home Negotiated Care Plans:
Available to adult family home providers for consultation and participation in the development of the mandated negotiated care plan. Fee for this service is usually paid for by the adult family home. Cost: $100.00
Private Case Management:
Assistance for those who are in need of a professional case manger to manage a crises situation or ongoing supervision of an individual's daily needs when family or friends are unavailable to offer support. $40.00/hr
Guardian ad Litem:
Ms. Johnson is a certified Title 11 Guardianship Guardian ad Litem and is on the Superior Court GAL registries for Thurston, Mason and Pierce Counties. Ms. Johnson is available for appointment based on the rotation system policies in each county.
